1. Details of Applicant/ਬਿਨੈਕਾਰ ਦਾ ਵੇਰਵਾ
2. Details of Deceased/ਮ੍ਰਿਤਕ ਦਾ ਵੇਰਵਾ
3. Names of villages in which land of Deceased is situated /ਉਨ੍ਹਾਂ ਪਿੰਡਾਂ ਦੇ ਨਾਮ ਜਿਨ੍ਹਾਂ ਵਿੱਚ ਮ੍ਰਿਤਕ ਦੀ ਜ਼ਮੀਨ ਸਥਿਤ ਹੈ
4. Details of natural heirs of deceased/ ਮ੍ਰਿਤਕ ਦੇ ਕੁਦਰਤੀ ਵਾਰਸਾਂ ਦੇ ਵੇਰਵੇ
5. Details of Last will(if any) executed by the Deceased/ ਮ੍ਰਿਤਕ ਦੁਆਰਾ ਕੀਤੀ ਗਈ ਆਖਰੀ ਵਸੀਅਤ(ਜੇ ਕੋਈ ਹੈ) ਦਾ ਵੇਰਵਾ
